These terms are used to describe family members who are two generations removed from you.
Great-Grandmother
This refers to the grandmother of your parent.
Example: "My great-grandmother was born in the 19th century."
Great-Grandchildren
These are the children of your grandchildren.
Example: "I hope to meet my great-grandchildren someday."
Note: To indicate more distant relatives, you can add additional "greats" to the terms. For instance, the child of a great-grandchild would be a great-great-grandchild.
